We are seeking a warm, organized, and detail oriented First Impressions Coordinator to join our client focused care team. This person will play a key role in ensuring our clients and their families have a seamless, personalized, and consistently positive experience with Coastal Care Partners.
Key Responsibilities:
- Serve as a primary point of contact for our medical and home care clients.
- Proactively check in with clients to ensure satisfaction and quickly address any concerns
- Manage care transitions and updates, including changes in client condition or care plan
- Maintain accurate client records and communication notes in multiple systems
- Support the onboarding of new clients and help create personalized care experiences
- Collaborate closely with medical providers, leadership, caregivers and internal teams to ensure quality and consistency
