Key Responsibilities:

• Serve as a primary point of contact for our medical and home care clients.
• Proactively check in with clients to ensure satisfaction and quickly address any concerns
• Manage care transitions and updates, including changes in client condition or care plan
• Maintain accurate client records and communication notes in multiple systems
• Support the onboarding of new clients and help create personalized care experiences
• Collaborate closely with medical providers, leadership, caregivers and internal teams to ensure quality and consistency
